Chuck, the best way to figure out how to use the D string is the following.
First, are there any chords you play that require two feet where your right foot presses pedal 7. If so, you no longer need to use two feet. You can use your left foot and string one. One example is playing a D13 at the open fret with pedals 5,7. With string one, you only need to press pedal 5 and lower string 3 using the standard knee lever.
Second, you can now play a scalar run on strings 2,1,3 (similar to playing strings 3,1,4 on E9), providing better options for your soloing.
As far as first string D note pulls, some players, including BE, raise the string to a D#.
