Bruce - assuming you want to upload something on your computer - click the Post Reply button, then look below the reply text area for the attachment tab. Click it, click Add files, navigate to your files, select the image file you want to upload, and click the button to open the file.
This will bring up a new section on the reply page below the Attachment tab to either place the file inline or delete the file. Select Place inline. This will put the attachment inline to where your cursor is, and that looks like this:

This step is essential. If you want to move the attachment to a different place, just cut and paste that attachment code wherever you want it.

One more thing - once you have made your post, your photo attachment will be stored on the server, but not before that. Once it's on the server, it will have a URL from which you can reference it again using the img tags, which is what Insert Image does (and has always done). So if you have a photo that you'd like to post, with a valid URL, you can simply place the image inside the img bbcode brackets. For example, this code
