Sarno Musical Solutions V8 preamp. Included (and attached in the pic) is the rack shelf that Brad sells for these.
This one has the balanced XLR out as well as the adjustable master level on the 1/4" out. There is some 3M dual lock attached to the top that I put on at some point for attaching pedals.
Sounds amazing. Good for so many things.

I should also add that I went through a bunch of old octal preamp tubes and found a couple that worked really well with no microphonics. They are probably old GE's, though I don't remember. I'll also include the 2 stock JJ 6SL7's that came with it and still work well. I guess it's a good deal now!
