Another thought is that the wrap on the string may be lifting the string just enough to where it doesn't contact the top of the finger. Some strings have a long wrap and some short.

Tell us about your amp and settings you're using. It looks like your playing a Carter but the picture isn't very clear.If it is, I had the best tone using an XR-16.

You could grind a small flat where the set screw contacts the shaft and that should stop it from trying to rotate around the shaft.

I had one on my seat but took it off. It added more weight and I found I wasn't leaning on it at all because it's placed just far enough back to where I had to scoot back so it would touch my back.
