On the ten string E9 tuning 3 floor pedals and 5 knee levers is very common. 4 floor pedals and 5 knee levers is also getting to be popular. That's what I have.

I answered "other" because I also have 5 knee levers, which as Paul suggests is close to becoming standard.

Other. Mine's a 4x5. I, too, was surprised 5 KLs was not a stand-alone option.

3x5 on my S10 Sho-Bud
5x5 on my Mullen S12
9th string D has been removed from both guitars, so Bud S10 goes down to G#
E's on left on both guitars

other:

3 and 5, while I routinely use 3 + 4, the 5th lever, the V lever , lowers 6 while the LKL raises 1,2 and 7. I do not lower 5 or the 5+10 combo. Never did.

My Carter D10 (9p/9k) has 5p/6k On E9,so I guess that makes me an "other". I just received my new Mullen SD12 on Wednesday. It is 3p/4k. I'm trying to get back to basics with the pedals and levers I use most. That doesn't mean I won't add more later.
