Luthier John Monteleone is revered in the jazz guitar community for his art deco-inspired arch tops. This FJ post talks about Ben Harper's commission for Montelone's first-ever lap steel (which looks pretty much like a typical monteleone jazz guitar with a raised nut). Looks like the controls are on the tailpiece.
Great exposure for our instrument!

You probably won't see this pic anywhere else (I received it in January, but the word was the guitar was actually in the finishing stages then), but here's a closeup of the guitar in progress:
